After our initial consultation, we put together a plan of prenatal classes to prepare you and your partner for the journey ahead.

All of the classes are done in-home and are customizable to meet your family's needs and preferences.

Our visits are never time limited, we stay as long as you need us for.

In the rare event that your child needs to go to the emergency room, Dr. Weems will support you through every step of the way and will be in close contact with the hospital’s medical staff and help coordinate your care.

We understand that families move as they grow. As such, we try our best to continue care at whichever suburb your family settles in or we will make sure to set you up with a practice to match your needs.

We highly encourage teenagers to have their own accounts with us because we know the importance of empowering teens to take control of their own healthcare.

Of course, this is only done with parental permission and the parents will have accounts as well.

While our model of care is not supported by insurance companies, this allows us to do so much more- longer visits, more frequent visits.

Some HSA and FSAs can be used to cover your membership, this is worth looking into.

We will still utilize your insurance for things like tests and vaccines. We also can provide “super bills” that can be submitted to your insurance company that go towards your out of network deductible. Insurance can be very confusing, we would love to talk more about this during our initial consultation.

Do not worry about this at all! It is our job to get these records and comb through them so we can formulate the best, most customized plan for your family.

Our pricing model was created based on the fact that as kids get older, they need to be seen less and less by their pediatrician. You will notice that your membership price decreases as your kid gets older.

We are happy to offer family discounts so that your growing family can all receive their healthcare from CCP.

Dr. Weems has privileges at Northwestern Medicine Prentice Women’s Hospital, Northshore’s Evanston Hospital, and Advocate Hospitals. If you are delivering at one of those hospitals, she will see you everyday that you are in-patient.

Since Dr. Weems has been doing this for so long, she has deep connections at every hospital in the greater Chicagoland area and is always in constant communication with your child’s specialist or hospitalist, etc.

Yes, we offer a fee-for-service for all non-members although we strongly encourage everyone to join for continuity of personalized care.

This mode of healthcare is recommended for those visiting from out of town, when an unexpected illness strikes, or those who simply need a doctor to come over for an in-home, untimed visit.
